Grandam Hornsent is another NPC whose questline you will get to complete in Elden Ring Shadow of the Erdtree. Like with most side quests in the game, this one too is rather extensive and you will be able to round it out only after beating Messmer, the Impaler.
However, to start Grandam Hornsent’s quest you will need to meet her first. The NPC is pretty hard to find, and the quest is entirely missable if you are unable to locate her in Belurat.

Where to find Hornsent Grandam in Elden Ring Shadow of the Erdtree?
You will find Hornsent Grandam in Belurat behind the locked Storeroom Door. You will need the key to unlock it and you will be able to find it near the Small Private Altar Site of Grace.
From the Grace head up they stared and took out the Horned Warrior. Then return left and you will find yourself at the top of the waterfall that goes down to the fountain area below.
Make your way north and you will find an alleyway that leads to the other side. You will find Shadow Undead along the way, but move past them and take a left at the river. You will then be led into a building on the right.
Climb the ladder there and you will reach the top. Here you will find a corpse, so loot it to get the Storeroom Key.
Once you have the key, go back to the Storeroom to unlock the door and talk to Hornsent Grandam to start her questline.
How to complete the Hornsent Grandam questline in Elden Ring Shadow of the Erdtree
After the initial exchange of dialog here is what you will need to do in order to complete the Hirnsent Grandam questline:
1) Beat the Divine Beast Dancing Lion
The first part of th quest will require you to defeat the Divine Beast Dancing Lion. Once you take it down, you will receive the Divine Bet Head helmet.
Wear the item and then head back to Hornsent Grandam. Talk to her and there will be a new exchange of dialogs. Once you exhaust everything that she had to say, you will receive the Wathful Spirit spell.
Reset the game by resting at a Site of Grace and then talk to her again. This time she will give you a Scorpion Stew.
2) Give Hornsent the Scorpion Stew
You will find Grandam’s son Hornsent right after Castle Ensis at the Highroad Cross Site of Grace (this will be the second location he travels to). Interact with him and give him the Scorpion Stew and there will be new dialog’s.
You can summon this NPC in a boss battle, however, choosing to do it or to not do it will not affect the questline in any way.
3) Defeat Messmer, the Impaler
For the final part of the questline, you will need to beat Messmer, the Impaler in Shadow Keep. So defeat the boss, and then make your way to Hornsent Grandam.
You don’t need to put on the Lion helmet again this time around. Talk to her and she will give you the Gourmet Scorpion Stew which is an item that will restore your health over time.
After you exhaust all of her dialogs she will then doze off. You get to talk to her again right after you beat the final boss of FromSoftware’s latest DLC.
